Compassionate Care for Your Loved Ones!

Community Integration & Independent Living Support

Community Integration & Independent Living Support

Life feels richer when home skills and community participation grow together. Our Community Integration & Independent Living Support is designed for adults of all abilities who want to build confidence at home and become more engaged in everyday community life.

We start with independent living skills training in the home-practicing routines like preparing simple meals, managing laundry, organizing personal belongings, and following a daily schedule. Caregivers coach step-by-step, breaking tasks into manageable parts and using prompts or visual supports when helpful. The goal is not to do everything for the person, but to encourage them to do as much as they safely can.

Community access support builds on those skills. We accompany clients to local activities, libraries, parks, stores, job sites, or day programs, focusing on real-world skills like using transportation, handling money, communicating needs, and following social expectations. For individuals receiving developmental disabilities support, this can be a key part of person-centered planning.

Because each person's goals are unique, we develop customized care plans that align with interests and strengths-whether that is learning to cook a favorite recipe, joining a community class, or becoming more comfortable speaking with others. We collaborate with families, case managers, and, when appropriate, our Adult Foster Care homes to ensure consistency across settings.

Support can be scheduled a few hours a week or more frequently, depending on the level of guidance and supervision needed. Over time, many clients experience more independence, stronger self-advocacy, and a deeper sense of belonging in their neighborhoods.

If you or your loved one is ready to build independence while staying safe and supported, we are here to help. Contact us today for a free consultation to explore community access assistance and home-based skill-building.
